Els edemes cerebrals s\u00f3n acumulacions de l\u00edquid als espais intra o extracel\u00b7lulars del cervell i poden ser conseq\u00fc\u00e8ncia de diversos factors com un accident cerebrovascular o un traumatisme craneoencef\u00e0lic, entre d\u2019altres. L’edema cerebral \u00e9s un problema seri\u00f3s en neurologia. Mentre que en altres \u00f2rgans un inflament no condueix a una situaci\u00f3 urgent, en el cervell porta al coma i posteriorment a la mort. Encara que hi ha solucions terap\u00e8utiques com la cirurgia , es requereixen tractaments m\u00e9s efectius.<\/p>\n

La leucoencefalopatia megaloencef\u00e0lica amb quists subcorticals (MLC) \u00e9s un tipus rar de leucodistr\u00f2fia (afectaci\u00f3 de la mat\u00e8ria blanca) d\u2019origen gen\u00e8tic. MLC es pot considerar com un model d\u2019edema cr\u00f2nic, ja que els pacients afectats presenten des de el moment de naixement una elevada acumulaci\u00f3 d\u2019aigua<\/p>\n

Un estudi sobre la fisiopatologia d\u2019aquesta malaltia rara ha perm\u00e8s descobrir un dels mecanismes que desestabilitza l\u2019equilibri homeost\u00e0tic de les c\u00e8l\u00b7lules del cervell provocant edemes. Aquest estudi s\u2019ha publicat en el darrer n\u00famero de la revista Neuron<\/span><\/a>. La revista ha considerat que el treball era molt interessant i l\u2019ha acompanyat d\u2019un comentari de l\u2019editor i d\u2019un v\u00eddeo explicatiu.<\/p>\n

Investigadors de l\u2019IDIBELL i de la Universitat de Barcelona<\/span><\/a> (UB) i del CIBERER <\/span><\/a>(Centre de recerca en xarxa de malalties rares) han descobert que una de les funcions de la prote\u00efna GlialCAM, que est\u00e0 alterada gen\u00e8ticament en els malalts de MLC, \u00e9s regular l\u2019activitat del canal que permet el pas d\u2019ions de clor entre c\u00e8l\u00b7lules del cervell per regular l\u2019equilibri d\u2019ions i l\u00edquid. Quan falta aquesta prote\u00efna, el canal no funciona correctament i s\u2019acumula l\u00edquid a les c\u00e8l\u00b7lules glials del cervell formant edemes.<\/p>\n

Ra\u00fal Est\u00e9vez, director d\u2019aquest treball i Virginia Nunes, col\u00b7laboradora de l\u2019estudi creuen que la import\u00e0ncia d\u2019aquest descobriment \u00e9s doble. \u201cPer una banda\u201d, ha explicat Virginia Nunes \u201cens permet con\u00e8ixer millor la fisiopatologia d\u2019aquesta malaltia minorit\u00e0ria\u201d i \u201cper l\u2019altra\u201d, continua Ra\u00fal Est\u00e9vez, \u201chem identificat un mecanisme que pot obrir les portes a tractaments basats en l\u2019activaci\u00f3 d\u2019aquest canal per restablir l\u2019equilibri homeost\u00e0tic i potser tractar els edemes cerebrals en general\u201d.<\/p>\n

Els dos investigadors coincideixen a dir que aquest cas demostra que la recerca d\u2019una malaltia rara que afecta una petita part de la poblaci\u00f3 pot servir de model per identificar mecanismes que permetin pensar en nous tractaments per a malalties m\u00e9s comunes.<\/p>\n

La leucodistr\u00f2fia MLC<\/strong><\/p>\n

La leucoencefalopatia megaloencef\u00e0lica amb quists subcorticals (MLC) \u00e9s un tipus rar de leucodistr\u00f2fia que apareix durant el primer any de vida i est\u00e0 caracteritzat per la macrocef\u00e0lia (grand\u00e0ria excessiva del cap). Uns anys despr\u00e9s apareix un lent deteriorament neurol\u00f2gic amb at\u00e0xia (defici\u00e8ncia de la coordinaci\u00f3 motora) i espasmes. Les t\u00e8cniques de resson\u00e0ncia magn\u00e8tica revelen inflamaci\u00f3 de la subst\u00e0ncia blanca cerebral i la pres\u00e8ncia de quists subcorticals, especialment en les regions temporal i anterior. Una altra de les caracter\u00edstiques de la MLC \u00e9s la formaci\u00f3 d\u2019edemes cerebrals.<\/p>\n

En el 75% dels pacients de MLC s\u2019han identificat mutacions en el gen MLC1, causants de la malaltia. Els participants de l\u2019estudi Ra\u00fal Est\u00e9vez i Virginia Nunes van identificar recentment un segon gen causant de MLC, el GlialCAM.<\/p>\n

En el present estudi han identificat precisament la prote\u00efna GlialCAM com a una subunitat del canal i\u00f2nic que permet al clorur entrar i sortir del cervell de manera que les c\u00e8l\u00b7lules poden regular el seu equilibri homeost\u00e0tic.<\/p>\n
