El diagn\u00f2stic de la fertilitat masculina es realitza habitualment a trav\u00e9s de l’observaci\u00f3 dels espermatozoides al microscopi. Per\u00f2 un semen amb qualitat normal no garanteix una fertilitat adequada. De fet, existeix una considerable proporci\u00f3 de casos d’infertilitat de causa desconeguda i hi ha dades que suggereixen que la funci\u00f3 an\u00f2mala de l’espermatozoide podria tenir un origen gen\u00e8tic o molecular.<\/p>\n

Un treball realitzat per cient\u00edfics de l’Institut de Recerca Biom\u00e8dica de Bellvitge (IDIBELL) i de la Fundaci\u00f3 Puigvert ha identificat una petjada d’expressi\u00f3 g\u00e8nica vinculada a taxes d’embar\u00e0s molt baixes en donants de semen amb qualitat seminal normal. L’empremta gen\u00e8tica detectada \u00e9s un marcador sensible de la baixa capacitat fecundant de l’espermatozoide. Els resultats de l’estudi s’han avan\u00e7at en l’edici\u00f3 digital de la revista Human Reproduction i la tecnologia desenvolupada ha estat protegida a trav\u00e9s d’una sol\u00b7licitud de patent europea. A m\u00e9s, l\u2019estudi va guanyar el segon premi a la millor comunicaci\u00f3 en el 29\u00e8 Congr\u00e9s de la Sociedad Espa\u00f1ola de Fertilidad (SEF) celebrat la setmana passada a Granada.<\/p>\n

El treball compara l’efici\u00e8ncia diagn\u00f2stica de fecunditat mesurada a partir del patr\u00f3 de l’expressi\u00f3 d’ARN (\u00e0cid ribonucleic) de l’esperma amb els par\u00e0metres d’avaluaci\u00f3 que es realitzen habitualment en les mostres de donants de semen utilitzades en inseminaci\u00f3 intrauterina. Els investigadors van analitzar mostres procedents de 68 donants solters, joves i sans, dels que a priori no es coneixia la seva fecunditat real.<\/p>\n

Els investigadors van analitzar el perfil d’expressi\u00f3 de 85 gens en el semen de donants amb diferents taxes d’embar\u00e0s producte de inseminacions intrauterines. La selecci\u00f3 d’aquests 85 gens es va realitzar a partir d’una empremta d’ARN d’homes f\u00e8rtils normals ja establerta en investigacions anteriors. A partir d’aquesta an\u00e0lisi, es van trobar difer\u00e8ncies significatives en l’expressi\u00f3 de vuit gens individuals entre les mostres amb pitjors i millors taxes d’embar\u00e0s. Partint d’aquesta base, es va trobar que combinant els resultats de l’expressi\u00f3 de quatre d’aquests gens s’obtenia una sensibilitat molt major que amb l’an\u00e0lisi cl\u00e0ssica de semen (82% davant 23%) per recon\u00e8ixer els individus subf\u00e8rtils. El model va ser validat en una mostra independent de donants.<\/p>\n

L’estudi ha estat coordinat per la investigadora del grup de Gen\u00e8tica Molecular Humana de l’IDIBELL, Sara Larriba, i el metge i investigador del servei d’Andrologia de la Fundaci\u00f3 Puigvert, Llu\u00eds Bassas. Tots dos coincideixen a destacar que els resultats de l’estudi \u201cobren la porta a avan\u00e7ar en el coneixement de les causes de la infertilitat d’origen desconegut i a desenvolupar en el futur un test addicional per identificar individus de fertilitat baixa tot i tenir uns valors seminals normals. Aquest podria aplicar-se tant per a la selecci\u00f3 de donants de semen com per al diagn\u00f2stic de la infertilitat masculina\u201d<\/p>\n
