El c\u00e0ncer de mama \u00e9s el m\u00e9s freq\u00fcent en dones. Una de cada nou dones patir\u00e0 un c\u00e0ncer de mama al llarg de la seva vida. Els progressos en la prevenci\u00f3 i la detecci\u00f3 preco\u00e7, aix\u00ed com l’\u00fas de la quimioter\u00e0pia despr\u00e9s de la cirurgia (quimioter\u00e0pia adjuvant), han aconseguit en els \u00faltims deu anys incrementar significativament la superviv\u00e8ncia en aquesta malaltia, per\u00f2 encara queda molt per fer. <\/p>\n

La identificaci\u00f3 de pacients amb c\u00e0ncer de mama d\u2019alt risc \u00e9s clau per saber si una pacient requerir\u00e0 \u00fanicament l’extirpaci\u00f3 del tumor o si ser\u00e0 necess\u00e0ria la quimioter\u00e0pia addicional per estar segurs de l’eliminaci\u00f3 de les c\u00e8l\u00b7lules canceroses mam\u00e0ries. Actualment, es determinen les mutacions gen\u00e8tiques i patrons d’expressi\u00f3 ja coneguts, per\u00f2 el trencaclosques de la gen\u00e8tica de la malaltia roman en bona part inacabat. <\/p>\n

El director del Programa d’Epigen\u00e8tica i Biologia del C\u00e0ncer (PEBC) de l’Institut d’Investigaci\u00f3 Biom\u00e8dica de Bellvitge (IDIBELL), Professor de Gen\u00e8tica de la Universitat de Barcelona i Investigador ICREA, Manel Esteller, ha establert els patrons epigen\u00e8tics del c\u00e0ncer de mama i tamb\u00e9 les seves conseq\u00fc\u00e8ncies cl\u00edniques. La troballa es publica a la revista Molecular Oncology. <\/p>\n

“Hem analitzat les alteracions epigen\u00e8tiques, concretament el senyal qu\u00edmic anomenat metilaci\u00f3 de l’ADN, presents en 500 tumors de mama i hem comparat els patrons obtinguts amb el comportament cl\u00ednic d’aquests c\u00e0ncers” explica Esteller. <\/p>\n

\u201cObservem que hi ha dos subgrups de tumors de mama segons el seu epigenoma: un que hem anomenat Epi-Basal, caracteritzat per la p\u00e8rdua de marques epigen\u00e8tiques que causa trencaments en els cromosomes, i un altre que hem anomenat Epi-Luminal B, definit per presentar inactivaci\u00f3 epigen\u00e8tica de gens importants que ens haurien de protegir del c\u00e0ncer i en aquestes c\u00e8l\u00b7lules alterades ja no poden fer-ho\u201d. <\/p>\n

Segons l\u2019investigador el m\u00e9s destacat de la troballa \u201c\u00e9s que el subtipus Epi-Luminal B es comporta de forma especialment agressiva, i est\u00e0 associat a una superviv\u00e8ncia menor de les pacients. Aix\u00f2 pot ser \u00fatil per recomanar que en aquests tumors no ens conformem amb la cirurgia i determinar que probablement ser\u00e0 necessari administrar quimioter\u00e0pia complement\u00e0ria. Dit d’una altra manera, en aquells tumors amb patr\u00f3 epigen\u00e8tic m\u00e9s ‘benigne’, la cirurgia podria per si sola ser curativa; evitant aix\u00ed els efectes secundaris de la quimioter\u00e0pia” conclou l’investigador. <\/p>\n
