El sistema vestibular es troba a l\u2019o\u00efda interna i \u00e9s l\u2019encarregat del sentit de l\u2019equilibri. Una de les causes de la degradaci\u00f3 funcional d\u2019aquest sistema \u00e9s l\u2019exposici\u00f3 a agents qu\u00edmics otot\u00f2xics, que provoquen la p\u00e8rdua de les c\u00e8l\u00b7lules ciliades que el formen. Ara, un equip de la Universitat de Barcelona i l\u2019IDIBELL, en col\u00b7laboraci\u00f3 amb investigadors de la Universitat de Girona, ha determinat la causa de la recuperaci\u00f3 d\u2019aquest sistema en casos d\u2019otoxicitat cr\u00f2nica.<\/p>\n

Tal com explica Jordi Llorens, investigador del Departament de Ci\u00e8ncies Fisiologiques II i cap del grup Bases cel\u00b7lulars i moleculars dels trastorns sensorials de l\u2019IDIBELL, que ha liderat la recerca, un dels resultats principals del treball \u00e9s que \u201cs\u2019ha pogut observar que els s\u00edmptomes de p\u00e8rdua de la funci\u00f3 vestibular apareixen abans que tingui lloc la mort de les c\u00e8l\u00b7lules ciliades\u201d. \u201cConcretament \u2014continua l\u2019investigador\u2014, hem vist que, pr\u00e8viament, es produeix una desconnexi\u00f3 entre les c\u00e8l\u00b7lules ciliades i els terminals neuronals, i hem pogut mostrar com aquesta desconnexi\u00f3 \u00e9s, en bona part, reparable\u201d.<\/p>\n

Les causes principals d\u2019ototoxicitat s\u00f3n els antibi\u00f2tics de la fam\u00edlia dels aminoglic\u00f2sids, com ara l\u2019estreptomicina, o productes anticancerosos com ara el cisplat\u00ed. El que se sabia fins ara \u00e9s que l\u2019exposici\u00f3 a aquests compostos causa la mort de les c\u00e8l\u00b7lules ciliades, que s\u00f3n c\u00e8l\u00b7lules sensorials i transductors encarregats de detectar el moviment i transformar-lo en un senyal nervi\u00f3s. La mort d\u2019aquestes c\u00e8l\u00b7lules \u00e9s irreversible at\u00e8s que no hi ha regeneraci\u00f3. D\u2019altra banda, s\u2019havia observat en pacients que hi ha recuperaci\u00f3 funcional amb un alt grau de variabilitat \u2014que pot arribar a la recuperaci\u00f3 total\u2014 despr\u00e9s d\u2019eliminar l\u2019agent t\u00f2xic; per\u00f2 es desconeixia la causa d\u2019aquesta recuperaci\u00f3.<\/p>\n

Les c\u00e8l\u00b7lules ciliades estan embolcallades pels terminals neuronals, que formen una estructura similar a un calze. En el treball, publicat a la revista Disease Models and Mechanisms, s\u2019ha identificat un efecte inicial de la toxicitat cr\u00f2nica: l\u2019eliminaci\u00f3 de les unions \u2014que els autors han anomenat calicials\u2014, que hi ha entre la c\u00e8l\u00b7lula i el terminal neuronal. A escala molecular, aquest canvi estructural ha estat corroborat mitjan\u00e7ant dades immunohistoqu\u00edmiques que mostren una reducci\u00f3 significativa de prote\u00efnes d\u2019uni\u00f3 CASPR1 en aquesta uni\u00f3 que es recupera en eliminar l\u2019agent t\u00f2xic. Altres prote\u00efnes, com el canal de potassi KCNQ4, mostren una distribuci\u00f3 anormal com a conseq\u00fc\u00e8ncia de la toxicitat, la qual cosa comporta una alteraci\u00f3 de les propietats funcionals del terminal nervi\u00f3s.<\/p>\n

Actualment no existeixen tractaments per a les malalties vestibulars, tot i que alguns estudis apunten que poden afectar m\u00e9s de la meitat de la poblaci\u00f3 de m\u00e9s de seixanta anys. Segons Llorens, \u201ccomprendre les bases cel\u00b7lulars i moleculars de la degradaci\u00f3 per ototoxicitat i la possible reparaci\u00f3 posterior pot proporcionar dianes per a f\u00e0rmacs que aturin el proc\u00e9s de degradaci\u00f3 o accelerin la reparaci\u00f3 del sistema\u201d. Aquestes dianes tamb\u00e9 poden ser rellevants per a altres malalties vestibulars o per endarrerir la p\u00e8rdua sensorial relacionada amb l\u2019edat. La recerca tindr\u00e0 continu\u00eftat gr\u00e0cies a un ajut de la Societat M\u00e9ni\u00e8re.<\/p>\n

En l\u2019estudi, els autors han utilitzat un model animal en rates per estudiar la progressi\u00f3 de la disfunci\u00f3 vestibular i la seva reversibilitat durant i despr\u00e9s de l\u2019exposici\u00f3 t\u00f2xica amb nitrils. Encara que en humans no es produeix una exposici\u00f3 significativa a aquests compostos, els efectes s\u00f3n similars als que es produeixen en l\u2019exposici\u00f3 a aminoglic\u00f2sids en humans.<\/p>\n

El sistema vestibular es pot considerar un sis\u00e8 sentit del qual som poc conscients. Est\u00e0 localitzat a l\u2019o\u00efda interna, situat darrere del timp\u00e0. A cada o\u00efda tenim cinc epitelis sensorials vestibulars que estan formats, d\u2019una banda, per tres crestes ubicades als canals semicirculars i encarregades de detectar els moviments de rotaci\u00f3 del cap, i de l\u2019altra, per un utricle i un s\u00e0cul, encarregats de detectar la gravetat i els moviments lineals del cos.<\/p>\n

Encara que no en siguem conscients, el nostre sistema vestibular est\u00e0 calculant constantment el sentit de la gravetat per tal de corregir la postura del cos, i tamb\u00e9 s\u2019encarrega de fixar la mirada quan el cos est\u00e0 en moviment. La degradaci\u00f3 d\u2019aquest sistema, poc estudiat i sense tractaments efectius, pot provocar la p\u00e8rdua de l\u2019equilibri i com a conseq\u00fc\u00e8ncia m\u00e9s caigudes, aix\u00ed com vert\u00edgens, entre altres trastorns.<\/p>\n
