El c\u00e0ncer de pr\u00f2stata \u00e9s el segon c\u00e0ncer m\u00e9s freq\u00fcent en homes. Actualment, el cribratge de c\u00e0ncer de pr\u00f2stata es basa en l’examen digital rectal i en la determinaci\u00f3 dels nivells de PSA (antigen espec\u00edfic prost\u00e0tic) en sang: nivells alts d’aquesta prote\u00efna estan associats a un major risc de tenir aquest tumor. Des que s\u2019ha implementat aquest m\u00e8tode de cribatge, s\u2019han reduit significativament el nombre de morts per aquest c\u00e0ncer, per\u00f2 encara presenta certes limitacions. Per exemple, en molts casos els valors elevats de PSA es poden deure a altres malalties de la pr\u00f2stata com ara infecci\u00f3, inflamaci\u00f3 o hiperpl\u00e0sia benigna, entre d’altres, Aix\u00ed doncs, l’an\u00e0lisi de PSA pot comportar un sobrediagn\u00f2stic d’aquest tumor, resultant en nombroses bi\u00f2psies innecess\u00e0ries. A m\u00e9s, els nivells de PSA no es correlacionen amb l’agressivitat del tumor, la superviv\u00e8ncia o la resposta als diferents tractaments, cosa que porta a tractaments excessius de tumors no agressius. En aquest context, s\u00f3n necessaris biomarcadors no invasius m\u00e9s precisos que complementin i optimitzin el cribratge de c\u00e0ncer de pr\u00f2stata.<\/p>\n

Publicacions anteriors han posat en evid\u00e8ncia que en els tumors de pr\u00f2stata es troben alterats els nivells d’RNA petits no codificants (small non-coding RNAs <\/em>o sncRNAs<\/em>), mol\u00e8cules que, encara que no es tradueixen a prote\u00efnes, regulen l’expressi\u00f3 gen\u00e8tica. Entre elles destaquen els miRNAs<\/em> i els tsRNAs<\/em>, tots dos amb gran potencial com a biomarcadors per al c\u00e0ncer de pr\u00f2stata. Ara, un estudi recent fet per investigadors de l’IDIBELL confirma la utilitat dels tsRNAs en semen com a biomarcadors per al c\u00e0ncer de pr\u00f2stata.<\/p>\n

“En estudis anteriors hav\u00edem desenvolupat test basats en els nivells de miRNAs en semen com a biomarcadors per al c\u00e0ncer de pr\u00f2stata; ara evidenciem que l\u2019alteraci\u00f3 dels nivells de tsRNAs en el tumor tamb\u00e9 es pot detectar i quantificar en aquestes mostres de semen” comenta la Dra Sara Larriba, investigadora principal en gen\u00e8tica molecular de la infertilitat masculina i malalties urogenitals a l’IDIBELL i l\u00edder d’aquest estudi, realitzat en col\u00b7laboraci\u00f3 amb el Servei d’Urologia de l’Hospital Universitari de Bellvitge,\u00a0 el Servei d’Andrologia de la Fundaci\u00f3 Puigvert i la Unitat de Gen\u00f2mica d’Alt Contingut i Bioinform\u00e0tica de l’Institut de Recerca Germans Trias i Pujol. Aix\u00ed doncs, els investigadors estan conven\u00e7uts que el semen pot considerar-se com a font d’informaci\u00f3 no nom\u00e9s per a la fertilitat masculina sin\u00f3 tamb\u00e9 per altres patologies que afecten altres \u00f2rgans de l’aparell reproductor, com la pr\u00f2stata.<\/p>\n

Per realitzar aquesta an\u00e0lisi, Adriana Ferre-Giraldo, investigadora de l’IDIBELL i primera autora del treball, explica que “a partir d’estudis de seq\u00fcenciaci\u00f3 a gran escala, hem seleccionat diferents tsRNAs candidats que poguessin reflectir la salut de la gl\u00e0ndula prost\u00e0tica per despr\u00e9s validar-los en semen. Creiem que els models obtinguts basats en tsRNAs en semen com a biomarcadors no invasius milloraran el diagn\u00f2stic cl\u00ednic del c\u00e0ncer de pr\u00f2stata en combinar-los amb les an\u00e0lisis de PSA”.<\/p>\n

En conclusi\u00f3, la combinaci\u00f3 de totes dues an\u00e0lisis podria constituir un abordatge m\u00e9s eficient per a la detecci\u00f3 no invasiva del c\u00e0ncer de pr\u00f2stata, complementant les eines de diagn\u00f2stic actuals, identificant nom\u00e9s els casos cl\u00ednicament significatius i evitant aix\u00ed el sobrediagn\u00f2stic i les bi\u00f2psies innecess\u00e0ries. A m\u00e9s, la disponibilitat i implementaci\u00f3 d’aquests biomarcadors aportaria un pron\u00f2stic m\u00e9s prec\u00eds de la malaltia, millorant la seguretat de la discriminaci\u00f3 entre els tumors indolents i els m\u00e9s agressius.<\/p>\n
