La reproducci\u00f3 assistida ha perm\u00e8s a la majoria de pacients inf\u00e8rtils engendrar un fill mitjan\u00e7ant l’a\u00efllament d’espermatozoides seminals viables i el posterior \u00fas de la t\u00e8cnica fecundaci\u00f3 in vitro amb injecci\u00f3 intracitoplasm\u00e0tica d’espermatozoides (FIV-ICSI).<\/p>\n

Els pacients azoosp\u00e8rmics<\/strong>, com a conseq\u00fc\u00e8ncia de l’abs\u00e8ncia d’espermatozoides al semen, nom\u00e9s es poden beneficiar d’aquest procediment amb els espermatozoides que s’obtinguin directament del testicle mitjan\u00e7ant una bi\u00f2psia. No obstant aix\u00f2, l’azoosp\u00e8rmia ocorre com a resultat de, o b\u00e9, una obstrucci\u00f3 en el tracte genital (azoosp\u00e8rmia obstructiva amb espermatog\u00e8nesi conservada), o d’una fallada greu en l’espermatog\u00e8nesi testicular (azoosp\u00e8rmia secretora) que presenta pocs o cap espermatozoide al testicle. Patir un o altre tipus d’azoosp\u00e8rmia determina l’\u00e8xit en l’obtenci\u00f3 d’espermatozoides de la bi\u00f2psia testicular per al tractament de reproducci\u00f3 assistida.<\/p>\n

Diagnosticar l’origen de l’azoosp\u00e8rmia mitjan\u00e7ant biomarcadors moleculars no invasius \u00e9s, per tant, de gran rellev\u00e0ncia abans de decidir un tractament de reproducci\u00f3 assistida adequat per a la parella, ajudant a evitar bi\u00f2psies testiculars innecess\u00e0ries per al tractament reproductiu. El plasma seminal cont\u00e9 una alta concentraci\u00f3 de microARNs, majorit\u00e0riament continguts en ves\u00edcules extracel\u00b7lulars, que es poden quantificar, cosa que els converteix en candidats a biomarcadors de diagn\u00f2stic\/pron\u00f2stic.<\/p>\n

En aquest estudi del grup de Gen\u00e8tica Molecular Humana de l’IDIBELL, liderat per la Dra. Sara Larriba<\/strong>, en col\u00b7laboraci\u00f3 amb el Dr. Bassas<\/strong> del Servei d’Andrologia de la Fundaci\u00f3 Puigvert, confirmen la utilitat cl\u00ednica del test que van desenvolupar pr\u00e8viament (Barcel\u00f3 et al., 2018), basat en la quantificaci\u00f3 dels nivells del microARN miR-31-5p en semen, com a biomarcador no invasiu per distingir l’origen de l’azoosp\u00e8rmia. Aquest test es pot utilitzar amb precisi\u00f3 diagn\u00f2stica, no nom\u00e9s a les ves\u00edcules extracel\u00b7lulars del semen com a font de mostra, sin\u00f3 tamb\u00e9 directament al plasma seminal, cosa que evita la purificaci\u00f3 de les ves\u00edcules extracel\u00b7lulars, proc\u00e9s que requereix de formaci\u00f3 i equipament que pot no estar disponible als laboratoris cl\u00ednics. L’\u00fas de plasma seminal total com a font anal\u00edtica facilitar\u00e0 l’\u00fas de miR-31-5p com a m\u00e8tode de diagn\u00f2stic no invasiu mitjan\u00e7ant un procediment t\u00e8cnic simplificat i, per tant, aquest enfocament permetr\u00e0 que aquest biomarcador seminal basat en microARN sigui m\u00e9s f\u00e0cilment aplicable a la cl\u00ednica.<\/p>\n
