L\u2019esclerosi lateral amiotr\u00f2fica <\/strong>(ELA) \u00e9s una malaltia neurodegenerativa que es designa com a espinal quan els primers s\u00edmptomes apareixen en columna, bra\u00e7os o cames, o b\u00e9 com a bulbar quan el deteriorament de les neurones s\u2019inicia en el bulb raquidi, situat a la base del tronc encef\u00e0lic. Els s\u00edmptomes de l\u2019afectaci\u00f3 bulbar s\u00f3n problemes en la parla i la degluci\u00f3. L\u2019inici bulbar de l\u2019ELA \u00e9s minoritari en relaci\u00f3 a l\u2019espinal, per\u00f2 els pacients tenen un pron\u00f2stic pitjor. Malgrat la diferenciaci\u00f3 en la manifestaci\u00f3 dels primers s\u00edmptomes de la malaltia, un 80% dels pacients d\u2019ELA acaben experimentant problemes d\u2019articulaci\u00f3 en la parla. El deteriorament de la parla pot comen\u00e7ar fins a 3 anys abans del diagn\u00f2stic de l\u2019ELA, fet que fa fonamental la detecci\u00f3 primerenca de l\u2019afectaci\u00f3 bulbar.<\/p>\n

Amb aquestes premisses, l\u2019investigador del CIMNE<\/a> Alberto Tena<\/strong> va comen\u00e7ar a desenvolupar aquest projecte, que parteix de l\u2019enregistrament i an\u00e0lisi ac\u00fastica de la pronunciaci\u00f3 de les 5 vocals de l\u2019alfabet per part de 45 pacients d\u2019ELA de l\u2019Hospital de Bellvitge i 18 persones de control. Amb l\u2019an\u00e0lisi ac\u00fastic de la pronunciaci\u00f3 de les vocals es van nodrir uns sistemes de machine learning<\/em> (intel\u00b7lig\u00e8ncia artificial a partir d\u2019aprenentatge supervisat) que han perm\u00e8s desenvolupar uns models o marcadors de veu amb resultats molt satisfactoris<\/strong> en la identificaci\u00f3 i diferenciaci\u00f3 dels participants a l\u2019estudi amb afectaci\u00f3 bulbar, els que no la tenien i el grup de control.<\/p>\n

\u201cTenim molt treball per davant encara, per\u00f2 els primers resultats que hem obtingut mostren que l\u2019afectaci\u00f3 bulbar es pot detectar amb models autom\u00e0tics abans que sigui perceptible per a l\u2019o\u00efda humana, i que es poden establir mesures objectives que facilitin un diagn\u00f2stic preco\u00e7 i prec\u00eds<\/em>\u201d, destaca Alberto Tena<\/strong>. Ara per ara fins a un 10% dels pacients d\u2019ELA no s\u00f3n diagnosticats correctament de la seva afectaci\u00f3 bulbar en un primer moment.<\/p>\n

De la seva banda, la Dra. M\u00f2nica Povedano<\/strong>, investigadora principal del grup de recerca en Malalties Neurol\u00f2giques i Neurog\u00e8niques<\/a> de l\u2019IDIBELL i responsable de la Unitat Funcional de Malaltia de Motoneurona de l\u2019Hospital de Bellvitge<\/a>, considera que la recerca obre la porta per aprofitar el gran potencial de la veu en el diagn\u00f2stic de l\u2019afectaci\u00f3 bulbar \u201ci alhora demostra el valor de la integraci\u00f3 d\u2019altres disciplines en el nostre treball. En aquest cas s\u00f3n els enginyers els que ens poden ajudar en el diagn\u00f2stic de patologies neurodegeneratives<\/em>\u201d, conclou.<\/p>\n

La continu\u00eftat del projecte passa per ampliar la base de dades amb m\u00e9s veus i nous par\u00e0metres d\u2019an\u00e0lisi que permetin resultats m\u00e9s precisos i adre\u00e7ats a millorar la identificaci\u00f3 de l\u2019afectaci\u00f3 bulbar en els pacients d\u2019ELA. El projecte de l\u2019an\u00e0lisi de veu per aprenentatge autom\u00e0tic per a la detecci\u00f3 de l\u2019afectaci\u00f3 bulbar en pacients d\u2019ELA \u00e9s tamb\u00e9 l\u2019eix de la tesi doctoral d\u2019Alberto Tena<\/strong>, que realitza a la Universitat de Lleida (UdL<\/a>) sota la direcci\u00f3 dels Drs. Francesc Clari\u00e0 i Francesc Solsona. El treball, en el qual tamb\u00e9 col\u00b7labora la Universitat Tecnol\u00f2gica de Tallin (Est\u00f2nia), ha estat publicat al Journal of Medical Internet Research<\/em><\/a>.<\/p>\n
